Episode 4.01 'My Old Friend's New Friend'
Air date August 31, 2004
Directed by Bill Lawrence
Written by Eric Weinberg
Plot Description
J.D. gets his first introduction to the new psychiatrist, Dr. Molly Clock (Heather Graham) while she is discussing a patient's meds with a nurse. When she asks his name he tells her it's Johnny instead of J.D. Since she commits names of people she meets to her memory immediately, Johnny sticks. Molly's initial meeting with Elliot is awkward at first, but they soon find that they have a lot in common. When she encounters Dr. Cox, he realizes he may have met his match in the quick wit department.
Meanwhile, J.D. is excited because it's his last week as a resident before he becomes a full-fledged doctor. What he wants most is Dr. Cox to give him his seal of approval, and a big hug. Later, Dr. Cox tells him that he will continue to teach and guide him even when he is officially a doctor.
A hug is definitely not something J.D. will be receiving from Elliot because their friendship is still shaky after he told her that he didn't love her. And since he still lives with Turk and Carla, Elliot is feeling left out.
Turk and Carla return to work from their honeymoon. Almost immediately, Carla makes some changes. Instead of ice cream, Turk is only allowed to eat Rice Dream in the new, uncomfortable chairs Carla bought. When Turk's car, Malik, blows up, she lets him make the decision on what to buy. She suggests that he buys a used Mini-Cooper. But instead of buying a car, Turk buys three Vespa's for himself, Carla and J.D. Even when he realizes how impractical they are, Turk refuses to return them until Carla can't take it anymore and blows up at him. They both learn that they will have to do a lot of compromising if they are to have a happy marriage.

Episode 4.02 'My Office'
Air date September 7, 2004
Directed by Gail Mancuso
Written by Matt Tarses
Plot Description
Now that their residencies are over, J.D., Elliot and Turk are moving on to the next phases of their medical careers. Elliot is starting a cross-sectional imaging fellowship, where she'll be looking at bone biopsies. J.D. thinks he's going to be named chief resident by Dr. Cox. But that outcome could possibly change since Elliot throws her name in for consideration at the very last minute.
Cox makes a surprise decision by naming both Elliot and J.D. joint chief residents. To add to their already tense relationship, they have to share an office the size of a tiny closet.
Meanwhile, Molly Clock uses Dr. Cox as a guinea pig for research on a paper she's doing on conflict resolution between hospital personnel. She also uses conflicts between J.D./Elliot and Turk/Dr. Cox, who can't agree on how to remove a lightbulb stuck in a patient's lower g.i. tract, and her own issues with Carla as examples in her paper.
The Janitor provides Turk and Cox with some important information to help them remove the lightbulb; Carla and Molly resolve their issues about being the "go to" person when other people need advice.

Episode 4.03 'My New Game'
Air date September 14, 2004
Directed by Ken Whittingham
Written by Gabrielle Allan
Plot Description
Lawyer Ted informs Dr. Cox and Jordan that due to a legal mix-up years ago, they are still legally married. Amazingly they are happy about it and go on with their lives as a happily married couple. But the cracks begin to show immediately when they realize they may really be together for the rest of their lives. All of their annoying habits come to the surface. Things go from bad to worse, so Jordan decides to take baby Jack and go stay with her mother. Molly points out to Cox that they were happier before their marriage was re-instated, so they should go back to that time. Just as Jordan's is leaving, Cox asks her for a divorce, which she happily grants.
J.D. is annoyed that he has to be referred to as the Co-Chief resident since he's sharing the position with Elliot. He and Turk have a falling out when Turk operates on a patient and she ends up dying. Turk had asked J.D. to arrange it so that Turk would get to do the operation instead of the Todd. J.D. was hesitant, but he did it anyway and then felt guilty when she died.
Molly and Dr. Cox work together on a patient, Miss Meyers, whose looks have improved since she had reconstructive plastic surgery after an accident. She's trying to adjust to the fact that she is now attractive. Miss Meyers decides that she wants to have surgery again to restore her rather unattractive looks. Cox disagrees at first, but Molly convinces him that it's for the best.

Episode 4.04 'My First Kill'
Air date September 21, 2004
Directed by Ken Whittington
Written by Tad Quill
Plot Description
J.D. asks Dr. Cox to give his residents a pep talk to boost their courage. But instead of giving a rousing speech, Cox tells them that all of them will most likely kill a patient (either through neglect or not reacting quick enough to something) during their residency. This of course creates a mild panic among the doctors. J.D. doesn't think he has directly caused a patient's death, but Turk, Elliot and other doctors, admitted that they all had. Dr. Cox proved to be the one exception. Now J.D. is afraid that he will kill a patient in the near future.
His patient, Mr. Daniels, needs to have fluid drained from his heart using a long, thin needle, but J.D. is afraid that he might kill him, so he tries to dissuade him from having the procedure.
Meanwhile, Elliot and Molly are becoming fast friends. Elliot even invites her to Karaoke night, even though she's a little shy about revealing her off-key singing and quirky movements. Elliot receives support from Molly when she has to present one of her patients, Dean Phillips, to the ethics committee. Daniel needs a heart valve replacement, but his heroin use would destroy any benefits the surgery would provide. Despite that, Elliot is willing to fight for him to get the surgery.
But Molly surprises her when she testifies that she doesn't think Dean should have the surgery because of his many drug relapses. Despite Molly's testimony, the committee votes to let him get the heart valve anyway. But, Dean ends up missing his rehab appointments and then just disappears completely.
Molly and Elliot make up and go to Karaoke later that night.
Carla hurts Turks feelings when she demands that Rowdy the dog be kept out of the apartment. Things get worse when she accidentally leaves Rowdy on the roof of her car and he flies off, landing on the ground somewhere. Luckily, the Janitor does taxidermy as a hobby and is able to get a replacement Rowdy.

Air date September 28, 2004
Directed by John Inwood
Written by Angela Nissel
Plot Description
After watching Molly handle Dr. Kelso, Elliot realizes that she's the mentor she's been looking for all along. She even gets up enough courage to tell off Dr. Cox. For once, he's speechless. But, instead of responding to Elliot, Dr. Cox takes out his anger on J.D. But, later, when Molly introduces Elliot to her boyfriend, who happens to be a car thief, Elliot begins to question Molly's judgment.
Carla gets her own dose of courage when she stands up to Turk. After nights of him coming to bed and waking her up, she demands that they go to bed at the same time each night. She even makes him leave his boys, who are watching a baseball game at their apartment, to go to bed with her. J.D. tries to get some respect from his residents, but they just ignore him.

Plot Description
J.D. gets a surprise visit from his brother Dan (Tom Cavanagh), who tells him that their father has died. Surprisingly, J.D. takes the loss better than his brother. When they return from the funeral, Dan takes a bath, but doesn't come out for a few days. He just lays in the tub, drinking one beer after another. J.D. tries to reach out to him, but Dan is too shaken by the loss. Dr. Cox actually tries to do something nice, so he takes over his patient load. He even tries to give J.D. a hug, but he ends up punching him in the face instead. Meanwhile, Dan is still holed up in the bathtub.
Later, Cox and Dan team up and ambush J.D. They put on football jerseys, get some beer and pretend to watch a football game. The real purpose is to get J.D. to open up about losing his father. But then, something surprising happens, Cox tells him that he's proud of him. J.D. and Dan, along with Dr. Cox, end up reminiscing about the good times with their dad.
Turk is dealing with a different type of loss. He's losing weight, very tired and has to go to the bathroom all the time. Carla can't get him to go to the doctor, so she uses Molly to get to him. She pretends that she has a patient with Turk's symptoms. Turk immediately speculates that the "patient" may have diabetes. He then realizes that she was referring to him. Turns out he has type II diabetes. He doesn't tell Carla that he has it. But he doesn't know that she already knows. She broke into Dr. Landow's office and looked at Turk's file. When Turk and Molly get a chance to talk, she tells him that he's afraid to be vulnerable with Carla because she sees him as her Superman, so he doesn't want to disappoint her.
Meanwhile, the Janitor plays a joke on Dr. Kelso by doing small things to make him think he's suffering from Alzheimer's disease.

Air date October 26, 2004
Directed by Zach Braff
Written by Mike Schwartz
Plot Description
Molly (Heather Graham) informs Elliot that she is moving to Milwaukee for a psychiatric fellowship. Elliot is really disappointed that she's leaving. J.D. shows his feelings differently. He and Molly end up making out after Molly's going away party. She later says that she's not romantically interested in him. He's just too normal for her. She changes her mind when he emphasizes some of his more negative and dysfunctional characteristics, which is the type of qualities that Molly likes in a man.
Molly decides that they can sleep together before she leaves, but J.D. has to get Elliot's permission first, since Molly and Elliot are friends. Elliot says yes, even though she and J.D. had been "even" since he broke up with her and then she slept with Dan. J.D.'s sleeping with Molly throws things off again.
Dr. Cox becomes a part-time EMT to fulfill Dr. Kelso's community service requirement. He teams up with Denise (Molly Shannon), the wacky, talkative ambulance driver. Driving with her is akin to torture.
J.D. tries to get to Molly's apartment before she leaves for the airport. But due to a series of mishaps and bad timing, he and Turk end up out in the middle of nowhere, with no way to get home. Ironically, they come across Denise's ambulance, which almost ends up running them over. Denise gets a broken collar bone, while J.D. and Turk have the life scared out of them. Cox's attitude towards Denise changes when he finds out that she lost her ten-year-old son to a car accident.
When J.D. finally reaches Molly, she's about to get into the cab. Before leaving, she hands him a note. It's from Elliot and says, "Now, we're even." J.D. is foiled again.

Plot Description
Kylie and J.D. have been dating for a month, but she still won't sleep with him, leaving J.D. very frustrated. Turk and Carla are still going through their wedding growing pains. Turk complains that Carla always blows things out of proportion. Speaking of relationships, Dr. Cox and Janitor have a bet that Janitor and Elliot will end up together. Cox bets the Janitor his Porsche; Janitor wagers his van.
To further complicate J.D.'s love life, Molly Clock (Heather Graham) returns to the hospital. She's only back to see one of her former psychiatric patients. J.D. gets another shock when he realizes that Turk is talking on the phone to his old college girlfriend, Rosanna. J.D. takes the phone from Turk and throws it out the window. The phone makes it way back to Carla, who asks Turk why he has three missed calls from his ex-girlfriend Rosanna. He accuses her of making a mountain out of a molehill, so Carla retreats. Re Kylie, Turk advises J.D. to have some fun with Molly and then when she leaves he can re-focus on Kylie. Sure enough, Molly asks J.D. if he wants to go out for a beer. He happens to be on the phone with Kylie when she asks, but J.D. accepts Molly's invitation anyway. And Janitor seems to be winning his bet with Dr. Cox after Elliot jumps into his arms and declares him her man. Sure enough, Cox gives Janitor the keys to his Porsche. But, Janitor has his own trick up his sleeve when he tells Elliot that Cox needs to see them on an actual dinner date.
Carla realizes that she can't stand Turk talking to his ex, so she demands that he end the relationship completely. He agrees, so she thinks. When Elliot finds out that the Janitor lied to her their date, she's upset, but when he tells her that she's the only one that treats him like a real person, she calms down.
J.D.'s night doesn't end as well. When he stops by Kylie's apartment, he mentions that he had the opportunity to be with another girl, but he held off because of his feelings for Kylie. She gets angry and throws him out. He tries calling her the next day, but she doesn't respond. When Carla finds out that Turk's ex just now found out that he's married, she's furious and storms out, leaving both J.D. and Turk without their women.
